Getting a free amplified telephone is easy
There are two ways to get your free amplified phone.
Here is how it works:
1) Go to one of the offices in our retail service area for a free hearing screening and to have your paperwork filled out for you. They will even mail it for you. To find the location nearest you simply type in your zip code in the space below:
2) Although you may live out of our service area we can still get you a free amplified phone. However, the process is a little different.
1. Fill out the registration form provided below. Then print out the relevant form for the product you wish to redeem.
2. Take it to a local hearing healthcare professional for their signature.
3. Mail it in to the address provided on the application along with proof of residency (ie: Texas drivers license).
4. Once your application is approved, STAP will mail you a large manilla envelope containing your yellow voucher. Keep the recipient copy and mail the unsigned voucher along with your choice of phone to:

5. Once we receive your voucher we will send you your phone. Note: signature required on the package delivery.
